COVID-19 vaccination event for CPS students and family members
Chicago Public Schools is excited to welcome its students and their families to receive the COVID-19 vaccine! All Chicagoans aged 5 and older are eligible to get vaccinated to protect against COVID-19. Beginning November 10, 2021, all CPS Regional Clinics and Mobile Events will also offer the vaccine for children aged 5-11 years. The vaccine is free, and no proof of insurance nor citizenship is required to get vaccinated.
Eligible patients (Minors under the age of 18 years) must be consented by a parent/guardian unless an exception applies and/or written consent has been provided through this registration process.
